Feedback & Concerns

The Ottawa Power Wheelchair Hockey League (OPWHL) is committed to maintaining a respectful, inclusive, and positive environment for all players, families, volunteers, and community members. Open communication is an important part of ensuring that our league continues to grow and improve.

If you have a concern or issue related to league activities, we encourage concerns to be addressed through the following process:

Step 1 — Speak with your coach

Many concerns can be resolved quickly through a conversation with your team coach.

Step 2 — Contact the League Coordinator

If the concern cannot be resolved at the team level, you may contact the League Coordinator directly at:

Step 3 — Submit a formal written concern

If the issue remains unresolved, you may submit a formal written concern using the OPWHL Feedback & Concerns Form.

Written submissions help ensure that concerns are clearly documented and reviewed appropriately by league leadership.

When submitting a concern, please include as much relevant information as possible, including:

• Date(s) of the incident or concern

• Team(s) or individuals involved

• A description of what occurred

• Any steps already taken to resolve the issue

• The outcome or resolution you are hoping for

Anonymous submissions are accepted; however, including your contact information may allow the league to follow up if clarification is needed.

All submissions will be treated respectfully and reviewed by league leadership. Submission of this form does not guarantee a specific outcome, but all concerns will be considered in accordance with OPWHL policies and the information available.
